Group L: Landmark guided Ankle block |
Ankle block will be performed by traditional landmark guided technique to block tibial, Superficial and deep peroneal, saphenous and sural nerves. Local Anesthetic solution total of 30ml consisting of 10 ml Inj Ropivacaine 0.75% +
10ml Inj Lignocaine+adrenaline 2%+ 10ml sterile Normal saline will be given to perform the block. Time of performing the block will be noted from once the needle enters the skin for
1st nerve block till the blocking of 5th nerve is completed this time will be noted. Duration of Analgesia and patient satisfaction score will be noted.
Duration to perform block will be noted rough estimation will be 20min prior surgery. Then followup duration will be 24hrs post surgery

Ankle nerves will be visualized under ultrasonography guidance and local anesthetic will be deposited to block tibial, Superficial and deep peroneal, saphenous and sural nerves. Local Anesthetic solution total of 30ml consisting of 10 ml Inj Ropivacaine 0.75% +

proximal tracing of Ankle nerves |
Using ultrasonography to scan the Tibial nerve, Saphenous nerve, Deep and superficial peroneal nerve, sural nerve which are commonly blocked in an ankle block. Using the ultrasound the course of these nerves will be traced proximal upwards in leg to find out the distances till which nerves can be easily seen and the measurement will be taken from fixed landmark points like Lateral and medial malleolus and intermalleolar line.

Brief Summary
|
This study is randomized, single blind, parallel group, single center trial will include 60 participants who are scheduled for elective foot surgeries under ankle block, will be randomized into two groups of 30 each where Group L will receive ankle block by landmark technique and Group U will receive ankle block under ultrasound guidance both these techniques are well established practice of ankle block. The primary outcome of this study to evaluate the effectiveness of these techniques in terms of Ease of performance, duration required, block success, analgesic requirement, total duration of analgesia, patient satisfaction feedback. the secondary objective of this study is to analyze the sono-anatomical correlation of Tibial nerve, saphenous nerve, superficial and deep peroneal nerve and sural nerve by proximal tracing of the these nerves by ultrasonography from fixed landmark points. |
