Brief Summary  

A COMPARATIVE CLINICAL STUDY TO EVALUATE THE EFFICACY OF NASYA AND UTTARABHAKTIKA SNEHAPANA WITH HRUSWAPANCHAMULA TAILA   IN THE MANAGEMENT OF AVABAHUKA VIS-A-VIS FROZEN SHOULDER.

Sneha is described as the essence of Purusha i.e, “Snehasro Ayam Purusha: Pranascha Snehabhuyishtaha1”. In Panchakarma, Snehana can be administrated either as Pradhana Karma (Brumhana Snehapana) and (Shamana Snehapana) or can be done as Poorvakarma (Shodhanartha Snehapana) before Shodhana Therapy. Brumhana kind of  Snehana can be administered as Panartha  and as well as Nasya , which  are found to be effective in  vatavyadhi2. Avabahuka is one among the vataja nanatmaja Vyadhi affecting Amsapradesha having symptoms like Shula, Sthamba, Bahupraspandanahara3.These symptoms resembles with the Symptoms of Frozen Shoulder. Frozen shoulder is characterized by pain and restricted movements of the shoulder usually in the absence of intrinsic shoulder disease4. The prevalence rate of Frozen shoulder globally is around 2-5%.In Indian population it ranges between 15-25%, among Subjects with Diabetes mellitus 3-6% encounter frozen shoulder5. Contemporary treatment for frozen shoulder includes oral NSAIDS, Corticosteroids, External therapies like Hot and cold compression packs6 gives symptomatic relief without targeting the root cause. In classics the treatment for Avabhahuka includes Nasya and Uttarabhaktika Snehapana7, Swedana8 and Nasapana9. Nasya and Uttarabhaktika Snehapana are the two important procedures which are specially indicated in Urdhvajatrugata Vatavikaras. Uttarabhaktika Snehapana is a type of Brumhana snehapana. Where administered Sneha immediately after food gives strength to Urdhvajatru Pradesha. The dose of Bruhmana Snehapana is Alpa matra10, Nasya is the therapeutic procedure in which the administered intranasal drug strengthens the Urdhvajatru Avayava and eliminate the aggravated Doshas11. Hence the present study aims to compare the procedural efficacy of Nasya and Uttarabhaktika Snehapana with Hruswapanchamula Taila12 which is directly indicated in Vata and Kaphanubandha Vatavyadhi. having the properties like Madhura, Brumhana and Vata-Kaphahara which counteract the pathology of the Avabahuka Vis-A-Vis Frozen Shoulder.
