Sciatic nerve catheter inserted for wound management in the hospital ward |
Sciatic nerve catheter will be placed in the sub paraneural compartment of the sciatic nerve above popliteal fossa for injecting local anaesthetic during wound dressing in the ward. The dose of local anaesthetic used would be 20 ml of 1.0 % lignocaine without adrenaline. Frequency of administration would be 30 minutes prior to dressing through the sciatic nerve catheter. The catheter will be removed when the pain score during dressing is less than 3 without local anaesthetic administration. |

Brief Summary
|
After getting written informed consent, patients will be educated about assessment tools used for pain , sensory and motor blockade and sciatic nerve catheter will be placed in the paraneural compartment under ultrasound guidance. The position of the catheter will be checked and patient will be shifted to ward. Thirty minutes prior to dressing, local anaesthetic solution will be injected through the catheter and degree of sensory and motor blockade will be assessed. After 30 minutes, wound dressing or debridement will be done in the ward treatment room. During the procedure, degree of analgesia will be assessed on a verbal numeric rating scale. After the end of the procedure, the recovery of sensory and motor conduction and duration of analgesia will be assessed at regular intervals. Catheter will be inspected everyday and if any signs of infection are found, catheter will be removed. Catheter will be retained until necessary based on wound healing and then removed. |
