| Intervention |
Group A: Pilates Breathing |
The subject will be positioned in supine, breathing freely without holding the breath. Hands will be placed over the lower posterolateral rib cage to guide breathing. The subject inhales through the nose and exhales through pursed lips while actively contracting the transverse abdominis and pelvic floor muscles. Transverse abdominis activation will be ensured by abdominal tightening and pelvic floor contraction, verified by palpation near the anterior iliac spine. Pilates breathing will be performed twice daily for one week, for 10 minutes per session. total duration of intervention is 7 days. Conventional exercise will be given. Diaphragmatic breathing-The patients will be placed in the semi fowler position and will be instructed to remain relaxed. Hand placement-One hand will be placed on chest and other on stomach. Upper Limb mobility- Upper limb mobility of shoulder, elbow and wrist will be given to patient. Each exercise will be given 10 reps for 5 times a week for 7 days. Lower limb mobility - Lower limb mobility of hip, knee and ankle will be given to patient. Each exercise will be given 10 reps for 5 times a week. Total duration of intervention is 7 days.

| Comparator Agent |
Group B: Papworth Breathing |
The subject is positioned lying down or sitting comfortably with good posture and eyes closed in a relaxed state. One hand is placed on the upper chest and the other on the upper abdomen to monitor breathing. The subject is instructed to relax the upper chest and allow the abdomen to expand during inhalation and gently fall during exhalation. Breathing is performed slowly through the nose, focusing on abdominal movement with minimal chest motion. A brief breath hold is introduced before slow, relaxed exhalation through the mouth. Attention is directed toward releasing bodily tension with each exhalation. The subject focuses on long, slow, deep breaths and maintains awareness of the breathing pattern. Distractions are acknowledged without judgment, and attention is returned to breathing. Papworth breathing is practiced twice daily for one week, with each session lasting 10 minutes. |

Brief Summary
|
This study will be conducted as a randomized clinical trial to evaluate and compares the effect of Pilates breathing and Papworth breathing in patient with moderate to severe Chronic Obstructive Pulmonary Disease. Eligible participants aged 45-65 years will be randomly allocated in two groups. Group A will receive Pilates Breathing exercise and Group B will receive Papworth breathing exercise along with conventional exercise for both groups. The intervention will be twice daily for one week. Pulmonary function will be assessed using spirometry. Functional capacity will be measured using the 6-Minute Walk Test. Oxygen Saturation will be evaluated using pulse oximetry. Respiratory symptoms will be assessed using Breathlessness, Cough and Sputum Scale. |
