Brief Summary   Pelvic ureteric junction obstruction is an important cause of obstructive uropathy in children and may lead to progressive renal damage if untreated Although pyeloplasty is the standard treatment postoperative assessment of renal recovery mainly relies on imaging methods such as ultrasound and diuretic renography which may not detect early functional improvement Matrix metalloproteinase 7 and tissue inhibitor of metalloproteinases 2 are involved in extracellular matrix remodeling inflammation and renal repair and may serve as urinary biomarkers for monitoring renal recovery This prospective observational study will include 30 children aged 1 month to 18 years with unilateral pelvic ureteric junction obstruction undergoing pyeloplasty at AIIMS Mangalagiri and Government General Hospital VijayawadaUrine samples will be collected at baseline intraoperatively from the renal pelvis and at 3 months and 6 months after surgery and the levels of MMP 7 and TIMP 2 will be measured using ELISA assays ultrasound diuretic renogram and estimated glomerular filtration rate will be used to assess renal functionStatistical analysis will compare biomarker levels before and after surgery and evaluate correlations with functional outcomesThe study is expected to demonstrate significant changes in urinary MMP 7 and TIMP 2 following pyeloplasty reflecting early renal remodeling and recoveryEstablishing these biomarkers may provide a non invasive method for monitoring postoperative renal improvement and may reduce dependence on repeated imaging
