1. Patients above 18 years, of both genders and diagnosed with chronic non-communicable diseases like T2DM, HTN, Asthma, Cardiovascular diseases, chronic respiratory diseases, chronic kidney diseases, and chronic liver disorder suffering from at least 6 months prior to study.
2. Patients with one or more comorbidities.
3. Patients attending either of the two selected PHCs
