Intervention / Comparator Agent
Type  Name  Details
Intervention  midline approach  After applying inclusion and exclusion criteria patients are grouped into paramedian approach or midline approach using randomisation technique For Patients in midline approach group midline of spine is identified in the transverse midline view using ultrasonography and interspinous spaces are marked. The interspinous space where the anterior and posterior complex are clearly visualised is chosen for the procedure. the angulation used while marking the space for appropriate view should be remembered to guide the needle while performing the procedure
Comparator Agent  paramedian approach  After applying inclusion and exclusion criteria patients are grouped into paramedian approach or midline approach using randomisation technique For Patients in paramedian approach group midline of spine is identified in the transverse midline view using ultrasonography and spaces are marked. corresponding interlaminar spaces are traced from the sacrum above in the parasagittal oblique view and marked. The interlaminar space where the anterior and posterior complex are clearly visualised is chosen for the procedure. the angulation used while marking the space for appropriate view should be remembered to guide the needle while performing the procedure

Brief Summary   this study aims to compare the midline vs paramedian approach of ultrasound assisted spinal anesthesia in elderly population posted for elective lower limb abdominal and perineal surgeries under regional anesthesia
ultrasound is an emerging and very useful modality perioperatively and becomes very useful in the elderly population with difficult anatomical landmarks of spine for administering anesthesia
various approaches are studied looking for the first pass success rate and number of passes as the outcomes mainly out of many other
