Intervention  Dexamethasone  Dexamethasone is a potent, long-acting corticosteroid used for its intense anti-inflammatory and immunosuppressant properties. 
Comparator Agent  Local Anesthesia   Lignocaine with adrenaline is the gold standard local anesthetic for dental infiltration due to its rapid onset 2–3 mins and intermediate action. It provides effective pain control for soft tissue procedures and restorative work

Pts who have taken analgesics or steroids 24 hrs before reporting.
Retreatment cases, Calcified canals, Vertical Root fracture

Brief Summary   This randomized placebo-controlled clinical trial aims to evaluate and compare the effectiveness of dexamethasone administered via different routes in reducing postoperative pain following biomechanical preparation in patients with apical periodontitis. Sixty systemically healthy patients aged 20 to 40 years with preoperative pain VAS  more than 5 will be randomly allocated into five groups: placebo calcium hydroxide with saline, calcium hydroxide mixed with dexamethasone, dexamethasone as an intracanal irrigant, dexamethasone as an adjunct with infiltration or block anesthesia, and dexamethasone via intraligamentary injection. Postoperative pain will be assessed using a Visual Analog Scale at 6, 12, 24, 48, and 72 hours. The study aims to determine whether the route of dexamethasone administration influences postoperative pain control following endodontic treatment in teeth with apical periodontitis.
