This randomized clinical trial aims to evaluate the impact of a Clinical Pharmacist Management Model on health outcomes and medication adherence among patients with cardiovascular disease attending a tertiary care teaching hospital. Cardiovascular disease requires long-term therapy, and poor adherence is a major contributor to treatment failure, complications, and repeated hospitalization. Clinical pharmacists play a key role in optimizing drug therapy, educating patients, and monitoring adherence. |
