Comparator Agent  Hyaluronic acid  Arthrocentesis as per Nitzan’s technique followed by injection of Commercially available non-crosslinked hyaluronic acid preparation (e.g., 1 mL) into the upper joint space. Clinical evaluation done at baseline, 1 week, 1 month and 3 months for pain, mouth opening and joint sounds.MRI evaluation of TMJ Disc and associated structures done at baseline and at 3 months. 
Intervention  Injectable Platelet-Rich Fibrin   Autologous blood collection 10ml and centrifugation using low-speed centrifugation protocol (e.g., 700 rpm for 3 min). Arthrocentesis as per Nitzan’s technique followed by 2ml i-PRF injection into the superior joint space under aseptic conditions. Clinical evaluation done at baseline, 1 week, 1 month and 3 months for pain, mouth opening and joint sounds.MRI evaluation of TMJ Disc and associated structures done at baseline and at 3 months.

Diagnosed with internal derangement of TMJ Wilkes stage two to four for whom conservative management has failed

Unilateral or bilateral involvement

Willing to provide informed consent and attend follow up visits 
 
ExclusionCriteria

Other systemic diseases that can affect the joint health

Previous TMJ surgery or injection

TMJ ankylosis

Wilkes stage one and five

Pregnancy or lactation.

Coagulation disorders or platelet dysfunction.

Brief Summary   This study compares the efficacy of injectable platelet rich fibrin and hyaluronic acid as intra articular injection in managing internal derangement of TMJ. Although there are very few studies with similar intent, the novelty of the study lies in objectively evaluating the Pre and Post intervention MRI scan images. Objective evaluation is necessary to know regarding the sustainability of the treatment outcome.
