Intervention  Supraclavicular brachial plexus block with perineural dexmedetomidine  It is a procedural intervention in regional anaesthesia and is a peripheral nerve block.Supraclavicular brachial plexus block will be done under the ultrasound guidance,20 ml of 0.5 percent Bupivacaine and 0.5 mcg per kg of Dexmedetomidine will be injected perineurally.It is compared with another group supraclavicular brachial plexus block along with intravenous Dexmedetomidine 
Comparator Agent  Supraclavicular brachial plexus block with intravenous dexmedetomidine  it is a procedural intervention in regional anaesthesia and is a peripheral nerve block.Supraclavicular brachial plexus block will be done under the ultrasound guidance,20 ml of 0.5 percent bupivacaine injected perineurally and 0.5 mcg per kg of dexmedetomidine diluted in 50 ml of normal saline will be administered intravenously over 30 minutes started immediately after the block.

Brief Summary   This prospective randomized comparative interventional study compares post operative analgesic efficacy of perineural dexmedetomidine and intravenous dexmedetomidine as an adjuvant to bupivacaine in supraclavicular brachial plexus block in elective upper limb surgeries.

The primary objective is to compare the duration of post operative analgesia using perineural dexmedetomidine and intravenous dexmedetomidine as an adjuvant to bupivacaine in ultrasound guided supraclavicular brachial plexus block in elective upper limb surgeries assesed by the time to first rescue analgesia.The secondary objective is to assess the total dose of rescue analgesics required in 24 hours in both groups.
