| Details |
Patients admitted in GICU of dept. of Anaesthesia IGMC Shimla and are on mechanical ventilator with following parameters
1. Normal performance status before disease onset ( per- illness frailty score less than 3)
2. Who are mechanically ventilated for at lest 24 h
3. With satisfactory clinical weaning parameters such as the ability to tolerate a spontaneous breathing trial (SBT) for 1 h
4. Stable haemolytic or on minimal and single vasopressin ,afebrile in the last 24 hours, with minimal tracheo bronchial secretions and effective cough response will be invluded |

3. Preexisting diaphragmatic palsy
4. Who underwent thoracic and abdominal surgeries
5. Where ultrasound is not possible due to wound dressings |

Weaning patients from ventilation has always been a challenging task and wearing failure , re intubation are associated with increased risk of morbidity and mortality.Diaphragm is the major respiratory muscle and its proper functioning is necessary for spontaneous breathing Considering diaphragm excursion and diaphragm thickness fraction along with clinical criteria and RSBI before weaning patients from ventilator has proven to decrease re intubation rates in some studies |
