Patients under ASA III/IV. Patient with facial fractures, cervical spine instability and patients requiring rapid
sequence intubation. Refusal to give consent
Patients with psychiatric illness and who are hemodynamically unstable. |

This prospective observational study evaluated the role of ultrasonographic airway parameters in predicting difficult laryngoscopy and endotracheal intubation in 105 ASA I–II adult patients undergoing elective surgery. Preoperative ultrasound measurements included tongue thickness (TT), distance from skin to epiglottis (DSE), hyomental distance (HMD), hyomental distance ratio (HMDR), distance from skin to hyoid bone (DSHB), and distance from skin to vocal cords (DSVC). Airway difficulty was assessed using Cormack–Lehane grading and Intubation Difficulty Scale (IDS). Results showed that difficult laryngoscopy and difficult intubation occurred in 36.2% and 28.6% of patients respectively. Tongue thickness and DSE emerged as the most accurate predictors (AUC > 0.85), with higher values significantly associated with difficult airway. Ultrasonographic airway assessment is a reliable, non-invasive, and objective tool that improves prediction of difficult airway, with TT and DSE being the most useful parameters for routine clinical practice.
