Day 0: Taking Demographic details
Week 1 Day 1: Explanation of central sensitization along with the use of pictures, visual metaphors and neurology pain questionnaires etc. Patients are encouraged to go through the content given through telerehabilitation.
Day 2-7:
1. Pain management
8-10 repetitions to each side
Relaxation ex.
Hot water fermentation
Gentle range of motion ex.
2.Mobility-exercise
8-10 repetitions to each side

3.Stretching- exercises 8-10 repetitions to each side

Week 2 Day 1 : Explanation of central sensitization along with the use of pictures, visual metaphors and neurology pain questionnaires etc. Patients are encouraged to go through the content given through telerehabilitation. The patient is encouraged to ask questions and receive clarification for any questions. Then addressing of the psychosocial aspects of the patient’s pain during any visit.
Day 2-7:

Patients must have a smartphone.
Pain duration- more than 3 months.
Individual diagnosed with radiating lower back pain due to PIVD and sciatica
Pain catastrophizing scale – Moderate to High level of Pain

Recent trauma or injury.
Severe cognitive impairment.
Recent spinal surgery and plans for upcoming surgery.
Presence of red flag signs [cancer, infection, fracture].
Pregnant females.
Use of strong opioids.

Brief Summary
|
This randomized controlled trial aims to evaluate the effectiveness of Pain Neuroscience Education (PNE) combined with conventional therapy compared to conventional therapy alone in patients with radiating lower back pain. After screening and informed consent, participants will be randomly assigned to two groups. The experimental group will receive PNE via telerehabilitation along with conventional physiotherapy; the control group will receive only conventional therapy.
Outcome measures will include pain intensity (NPRS), pain catastrophizing (PCS), functional disability (ODI), and fear of movement (TSK), assessed at baseline and after two treatment sessions over one week. |
