Intervention  Kegel Exercise  For 1 and 2 week - In 1 minute– 100 contractions then gradually increase to 3 minutes 300 and give rest of 1 minute between 100 contractions. For 3 week - In 4 min– 400 contraction and give rest of 1 minute between 100 contractions. For 4 week - In 4 min– 400 contraction and give rest of 1 minute between 100 contractions. 
Comparator Agent  Knack Exercise  For 1 and 2 week - supine lying position 8 repetitions for every session. For 3 and 4 weeks sitting and standing position 8 repetitions for every session.

Details  1.Vaginally delivered primiparous females.
2.Postpartum duration of 4 week to 8 week not participated in any type of exercise during pregnancy.
3.Invite the patients who learned how to correctly perform PFM contractions and with grade 2 muscle strength.
4.Patients who willing to participate.
5.Age 18- 30 years.
6.No previous pelvic surgery: No history of pelvic surgery that could affect muscle strength.

7.Pelvic floor muscle strength Grade 0 to grade 2 on perineometer assessment.
 
 
ExclusionCriteria 
Details  1.Postpartum women who reported with chronic diseases (hypertension, diabetes mellitus, chronic lung disease)
2.perineal tear grade III-IV,
3.subjects with cesarean delivery,
4.postpartum of preterm birth,
5.And complicated pregnancy or delivery.
6.Active urinary or vaginal infection in the past 3 months
7.Pregnancy: Currently pregnant or planning to become pregnant during the study.
8.Pelvic Floor Disorders: Diagnosed pelvic floor disorders (e.g., prolapsed, incontinence) requiring medical treatment.
9.Neurological Disorders: Any neurological condition affecting muscle function or coordination.
10.Severe Medical Conditions: Chronic conditions (e.g., cardiovascular, respiratory) that could limit exercise participation.
11.Use of Pelvic Floor Devices: Use of vaginal weights or other pelvic devices that could interfere with the study.
12.Subjects who are taking physiotherapy rehabilitation.

AIM :   To compare effect of four weeks of Knack Exercise verses Kegel Exercise on Pelvic Floor Muscle Strength among Postpartum Females.

OBJECTIVE -   1. To find out the effect of four weeks of knack exercises on pelvic floor muscle strength among postpartum females by using perineometer. 

 2. To find out the effect of four weeks of Kegels exercises on pelvic floor muscle strength among postpartum females by using perineometer.

 3. To compare the effect of four weeks of Knack exercises and Kegel exercises on pelvic floor muscle strength among postpartum females by using perineometer.

Procedure-   Ethical clearance will be obtained from Institutional Ethical Committee & of Dr. Ulhas Patil college of Physiotherapy. A written informed consent form will be obtained from subjects who are willing to participate. Subject will be screened according to inclusion and exclusion criteria. The aim of the study and its objective will be explained to willing participants. Selected participants demographics details outcome measures will be recorded.
Intervention Protocol-
GROUP A -  KNACK EXERCISE

Training protocol ( Week 1 and week 2)

 1. Exercise: Trunk flexion (detaching the shoulders from the support), contracting the vaginal muscles before and during the movement.

Initial position: Lying down with the knees bent.

Repetition: 8 repetitions for every session.

2. Exercise: Trunk flexion (detaching the shoulders from the support), placing the right hand on the left knee (diagonal) and contracting the vaginal muscles before and during the movement.

Initial position: Lying down with the knees bent.

Repetition: 8 repetitions for every session.

3. Exercise: Trunk flexion (detaching the shoulders from the support), placing the left hand on the right knee (diagonal) and contracting the vaginal muscles before and during the movement.

Initial position: Lying down with the knees bent.

Repetition: 8 repetitions for every session.         

Training protocol : 3 week

1. Exercise: Rise from a seat contracting the vaginal muscles before and during the movement.

Initial position: Sitting with the back straight and without any support.

Repetition: 8 times.

2. Exercise: Lift something from the floor contracting the vaginal muscles before and during the movement.

Repetition: 8 times.

3. Exercise: Squat contracting the vaginal muscles before and during the movement.

Initial position: Standing up.

Repetition: 8 times.

4. Exercise: Contract the vaginal muscles and cough twice. After coughing twice, relax the vaginal muscles. Repeat 3 times.

Training protocol : 4 week

1. Exercise: Walk or run in place contracting the vaginal muscles before and during the movement.

Initial position: Standing up.

Repetition: 20 to 30 seconds.

2. Exercise: Jump in place, contracting the vaginal muscles before and during the movement.

Initial position: Standing up.

Repetition: 8 times. 

3. Exercise: Squat and jump in place, contracting the vaginal muscles before and during the movement.

Initial position: Standing up.

Repetition: 8 times.

4. Exercise: Contract the vaginal muscles and cough 3 times. After coughing 3 times, relax the vaginal muscles. Repeat 3 times.

GROUP B - KEGEL EXERCISE

Training protocol: 1 week and 2 week.

Position: Patient must be in crook lying position.

Ask patient to tighten or contract your pelvic floor muscles and hold for 3-5 seconds initially. Relax the muscles for the same amount in given time.

Dosage: In 1 minute – 100 contractions then gradually increase to 3 minutes 300 and give rest of 1 minute between 100 contractions.

Training protocol:  3 week.

Position: Patient must be in crook lying position with pelvic bridging.

Ask patient to tighten or contract your pelvic floor muscles and hold for 3-5 seconds initially.   Relax the muscles for the same amount in given time

Dosage: In 4 min – 400 contraction and give rest of 1 minute between 100 contractions.

Training protocol:  4 week.

Position: Patient in squatting position.

Ask patient to tighten or contract your pelvic floor muscles and hold for 3-5 seconds initially.  Relax the muscles for the same amount in given time.

Dosage:  In 4 min – 400 contraction and give rest of 1 minute between 100 contractions.
