Brief Summary   SYNOPSIS FOR DISSERTATION CDPCHM 3 6 BRIEF RESUME OF INTENDED WORK 6.1 NEED FOR THE STUDY After a prescription has been made the physician commences to make observations about what changes occur in the patient and in which direction they are preceding. After long observation and keen examination of patients with his clinical mind Dr Constantine Hering first introduced the Direction of cure which pertains to the direction in which symptoms of the patient will disappear during cure under homeopathic treatment. There is very few literature available regarding direction of cure in Homeopathy. In Acute disease where beginners are so anxious to know the condition of the patient after administration of indicated homeopathic remedy by observing disappearance of symptoms into reverse order of their development under prescribed homeopathic remedy we can be assured that we are proceeding in the right direction it gives us patience and courage to wait for events knowing that prognosis is favorable and our work is effective as well. Herings direction of cure remains a relevant and useful tool for understanding healing process. It helps the physicians to decide prognosis of case and future plan of treatment. SYNOPSIS FOR DISSERTATION CDPCHM 4 6.2 REVIEW OF LITERATURE Dr Hahnemann by his fine observation and Inductive method of reasoning became convinced of law of cure that is SIMILIA SIMILIBUS CURANTUR and declared it to be NATURES LAW OF CURE APHORISM NO 26 1 In Homeopathy today Herings law of direction of cure is widely recognized as second law of cure. Herings law pertains to the direction of cure under homeopathic treatment. 2 At the time of cure the symptoms will disappear in following directions 1 From above downward 2 From within outward 3 More important to less important organs 4 In the reverse order of their appearance the first to appear being last to disappear. 3 Every homeopathic practitioner who understands the art of healing knows that the symptoms which go off in these directions remain away permanently. Moreover he knows that symptoms which disappear in the reverse order of their coming are removed permanently. 2 I summarize here the points most pertinent to the present discussion. 1 Dr Hahnemann 1828 In 1828 Dr Hahnemann published his first observations and theories on chronic diseases. All diseases acute and chronic of non venereal origin come from the original malady called psora. A skin eruption is the first manifestation of psora. The skin eruption acts as a substitute for the internal psora and prevents the breaking out of the internal disease. The more the skin eruption spreads the more it keeps the internal manifestations of psora SYNOPSIS FOR DISSERTATION CDPCHM 5 latent. But when the skin eruption is suppressed with an external application or other influences the latent psora goes unnoticed and its internal manifestation increases. Then it originates a legion of chronic diseases. Latent psora an abnormal susceptibility to disease will manifest itself as severe diseases after exposure to stress or as he calls it unfavorable conditions of life acute infections trauma and injuries exhaustion from overworking lack of fresh air or exercise frustration grief poor nutrition etc and by incorrect and weakening allopathic treatment. During the treatment of chronic diseases of non venereal origin with antipsoric remedies the last symptoms are always the first to disappear but the oldest ailments and those which have been most constant and unchanged among which are the local ailments are the last to give way. If old symptoms return during an antipsoric treatment it means that the remedy is affecting psora at its roots and will do much for its thorough cure. If a skin eruption appears during the treatment while all other symptoms have so far improved the end of the treatment is close. 4 This is the background that now leads us to Dr C Hering who among all Hahnemanns students was most similar to him. Like Dr Hahnemann Dr C Hering was a true scientist who totally adopted the inductive method in his work. 2 2 Dr C Hering 1845 In 1845 Dr C Hering published in the preface of the first American edition of Hahnemanns Chronic Diseases an extract of an essay which was never published elsewhere called Guide to the Progressive Development of Homeopathy. In this essay Hering writes Every homeopathic physician must have observed that the improvement in pain takes place from above downward and in diseases from within outward. This is the reason why chronic diseases if they are thoroughly cured always SYNOPSIS FOR DISSERTATION CDPCHM 6 terminate in some cutaneous eruption which differs according to the different constitutions of the patients. Hering enunciated the original observations of Hahnemann as a law of order in a work never to be published. 2 3 Dr C Hering 1875 In 1875 Dr C Hering published the first volume of Analytical Therapeutics of the Mind in which he stated that only such patients remain well and are really cured who have been rid of their symptoms in the reverse order of their development. 5 Here Hering makes no mention of the three other propositions regarding the direction of cure from above downwards from within outwards and from the more important to the less important organs. 2 Other stalwarts view related to Herings direction of cure. 1 Dr Herbert A Roberts In the acute manifestations the vital functions are often restored to complete harmony by and through their own power and it is noteworthy to observe the disappearance of symptoms in reverse order of their appearance. The first symptom to appear is last to disappear. Under a cure the symptoms of a deranged vital function always disappear in reverse order of their appearance. This is true in acute as well as chronic disturbances. Because this order is always maintained it is a vital point of observation in taking case to note the sequence of symptoms in invasion so that physician can know of progress of case under the selected remedy. 6 SYNOPSIS FOR DISSERTATION CDPCHM 7 2 Dr Stuart Close Cure takes place in definite orderly manner and direction. Symptoms disappeared from above downward from within outward and in the reverse order of their appearance. 7 3 Dr M L Dhawale Herings clinical observations in the treatment of patients suffering from chronic diseases led him to enunciate the Law of Direction of Cure which states that cure proceeds from within outwards from above downwards from more important organs to less important organs and in the reverse order of the onset the old suppressed symptoms becoming manifest in a mitigated form during this process of cure before they disappear for good. Clinical record should indicate unmistakably the chronological sequence of symptoms so that the Recent Symptoms could be evaluated. This chronological record is also of importance in evaluating the response in the light of Herings Law of Direction of Cure. Any deviation from this pattern will indicate a red light to a homeopathic physician who will have to take immediate measures to save the situation. 8 Dr Hahnemanns view on acute diseases as per Aphorism No 72 and 73 of 6th Edition of organon of medicine Definition Acute disease is that disease which is produced due to transient explosion of latent psora by some exciting cause Aphorism No 5 and 73 SYNOPSIS FOR DISSERTATION CDPCHM 8 Characteristics of acute disease Sudden onset Rapid progress Finish their course more or less quickly Duration of suffering is fixed and always moderate Without medicine terminates of itself either in recovery or in death Fate Without medicine it may lead to either recovery or death. If the disease brought on by exciting cause is not of very violent character then psora again returns to its latent state. But this happening always in sacrifice of some physical strength of patient. It requires proper medicinal help for its perfect eradication. 9 SYNOPSIS FOR DISSERTATION CDPCHM 9 6.3 OBJECTIVES OF STUDY 1 To observe and verify the direction of cure that symptoms disappear in reverse order of development in acute disease 2 Herings Direction of cure can be used as clinical measure to know prognosis of case after administration of suitable homeopathic remedy SYNOPSIS FOR DISSERTATION CDPCHM 10 7 MATERIAL AND METHOD 7.1 SOURCE OF DATA 1 OPD and IPD of Chandravatiben Dhansukhlal Pachchigar College of Homeopathic Medicine and Hospital near Navjivan Circle Udhana Magdalla Road Surat 2 Peripheral center OPD allotted by Chandravatiben Dhansukhlal Pachchigar College of Homeopathic Medicine and Hospital near Navjivan Circle Udhana Magdalla Road Surat 7.2 MATERIAL Materials utilized for study will be 1 Standard Case taking format of Chandravatiben Dhansukhlal Pachchigar College of Homeopathic Medicine and Hospital near Navjivan circle Udhana Magdalla road Surat 2 Homeopathic computerized software 3 Books all homeopathic and allied literature and source books related to my topic 4 Associated articles journals and websites if any 5 Any software for statistical analysis if required 6 Investigations as per requirement of case SYNOPSIS FOR DISSERTATION CDPCHM 11 7.3 METHOD OF COLLECTION OF DATA Study type Analytical study Study design Prospective study Study population Patients from OPD of C D Pachchigar Homeopathic Hospital and from peripheral OPD Surat Sample size 30 Sample techniques Simple random sampling Study duration 9 month SELECTION CRITERIA 1 Inclusion criteria Acute disease Patient between ages of 15 to 60 years Patient of both sex to be considered irrespective of their socioeconomic condition and religion Patients with their consent 2 Exclusion criteria Chronic disease and acute exacerbation of chronic disease Patient with structural pathological changes Patient suffering from autoimmune disease Emergency cases 3 Dropouts Cases with irregular follow up Case taking shall be done according to guidelines mentioned by Dr Hahnemann in 83 to 104 of Organon of medicine Then cases will be chosen on the basis of inclusion and exclusion criteria After proper analysis and evaluation of symptoms totality formation will be SYNOPSIS FOR DISSERTATION CDPCHM 12 done as per the instructions given by Dr Hahnemann in Organon of medicine The remedy will be selected on the basis of totality of symptoms and repertorization of selected rubrics with Homeopathic Software with final differentiation of medicine from Homeopathic Materia medica The remedies will be given in various potencies as per the requirements of the case The follow up will be taken as per requirement of case The remedies will be repeated as per requirement of case Change of medicine and or dosage as per the homeopathic principles after observing changes triggered after the administration of first remedy Homeopathic remedies will be dispensed from C D Pachchigar College and hospital Homeopathic pharmacy OUTCOME MEASURES Response will be analysed into 3 criteria 1 Significant improvement The significant decrease in the intensity and frequency of the presenting and associated complaints with a sense of wellbeing 2 Mild improvement The decrease in the intensity and frequency of presenting complaint with no sense of wellbeing 3 Status quo or no improvement No change will be seen in the symptoms neither increase or decrease in intensity or frequency of complaints Response as per Herings direction of cure 1 Symptoms disappear in exact chronological reverse order of their development will be considered as following Herings direction of cure during homeopathic treatment 2 Symptoms disappear but do not follow the exact chronological order of their development will be considered as not following Herings direction of cure SYNOPSIS FOR DISSERTATION CDPCHM 13 7.4 DOES THE STUDY REQUIRING ANY INVESTIGATION TO BE CONDUCTED ON PATIENT OR OTHER HUMAN OR ANIMALS Yes investigations done if required 7.5 HAS ETHICAL CLEARANCE BEEN OBTAINED FROM YOUR INSTITUTE Yes ethical clearance has been obtained from the institution SYNOPSIS FOR DISSERTATION CDPCHM 14 8 BIBILOGRAPHY 1 Das AK A Treatise on Organon of Medicine part 1 second edition Kolkata SouviK Kumar Das 104 Hemchandra Naskar Road Kolkata 700010 2007 2 Saine A Herings Law Law Rule or Dogma In Second Annual Session of the Homeopathic Academy of Naturopathic Physicians April 16-17 1988 Seattle Washington 3 Das AK A Treatise on organon of Medicine part 3 Kolkata Souvik Homoeo Publications 104 Hemchandra
