Title: A Comparative Clinical Study Between Kushmandak Rasayanam and Brahmi Ghrita in Vataj Unmada W.S.R. to ADHD (Attention Deficit/Hyperactivity Disorder)

Introduction:
Ayurveda emphasizes the connection between mind and body for health, where a balance is crucial for maintaining both physical and mental well-being. Attention Deficit/Hyperactivity Disorder (ADHD) is a neurobehavioral disorder in children, characterized by inattention, hyperactivity, and impulsivity, commonly seen in pediatric clinics. In Ayurveda, conditions like ADHD are understood through the lens of "Vataj Unmada" (a form of mental illness linked to the vitiation of Vata dosha). The study aims to assess the effectiveness of Ayurvedic treatments like Kushmandak Rasayanam and Brahmi Ghrita in managing ADHD symptoms.

Need for Study:
The prevalence of ADHD is rising globally, with studies indicating up to 11% of children in India affected by it. Conventional treatments using psychostimulants have associated side effects, which makes the need for safe and effective Ayurvedic alternatives essential. The study evaluates the efficacy of Brahmi Ghrita and Kushmandak Rasayanam in managing ADHD symptoms, as both are known to have beneficial effects on mental health.

Hypothesis:

  • Null Hypothesis (H0): Kushmandak Rasayanam and Brahmi Ghrita will not show effectiveness in managing Vataj Unmada (ADHD).
  • Alternative Hypothesis (H1): Kushmandak Rasayanam and Brahmi Ghrita will be effective, with one potentially being more effective than the other in managing ADHD.

Aims and Objectives:
The study aims to compare the effectiveness of these Ayurvedic treatments in ADHD. Specific objectives include:

  • Studying ADHD and its Ayurvedic correlation (Vataj Unmada).
  • Evaluating the effect of these treatments on ADHD symptoms.
  • Analyzing the physicochemical properties of both medicines.

Materials Used:

  • Ayurvedic Literature (e.g., Charaka Samhita, Sushruta Samhita, and other texts).
  • Modern Literature (e.g., Nelson Textbook of Pediatrics, DSM-V for ADHD criteria).

Drug Review:

  • Kushmandak Rasayanam is a combination of herbs including Kusmanda (Benincasa hispida), Pippali (Piper longum), and others, aimed at calming Vata and enhancing mental clarity.
  • Brahmi Ghrita contains Brahmi (Bacopa monnieri), Vacha (Acorus calamus), and other herbs, known for their calming effects on the mind, improving concentration and memory.

Posology:
Both treatments are administered orally with doses calculated using Young’s formula and given with lukewarm milk. The dose increases with the child’s age, starting from 3-5 grams and gradually increasing based on the child’s age.

Method:
The study is a prospective, interventional, open-label, non-randomized, double-arm clinical study, with 40 pediatric patients aged 4-12 years. The treatment is given for 60 days, with regular follow-up.

Inclusion Criteria:

  • Children between 4-12 years.
  • Diagnosis of Vataj Unmada (ADHD).
  • Parental consent for participation.

Exclusion Criteria:

  • Children under 4 years or above 12 years.
  • Children with chronic infections or seizures.

Assessment Criteria:

  • Subjective Criteria: ADHD symptoms are assessed using the DSM-V criteria, measuring inattention, hyperactivity, and impulsivity.
  • Objective Criteria: Conner’s Parent Rating Scale is used to evaluate ADHD severity.
  • Vataj Unmada symptoms are evaluated using Ayurvedic criteria for mental disorders.

Methodology of Treatment:

  • Group I will receive Kushmandak Rasayanam.
  • Group II will receive Brahmi Ghrita. Both treatments are assessed for their effects on ADHD symptoms and overall mental health.

Expected Outcomes:
The study aims to evaluate which of the two Ayurvedic treatments—Kushmandak Rasayanam or Brahmi Ghrita—is more effective in reducing ADHD symptoms. Potential benefits include improved concentration, reduction in hyperactivity, and better mental clarity, with no adverse side effects.

Conclusion:
This clinical study aims to provide insights into the effectiveness of Ayurvedic treatments like Kushmandak Rasayanam and Brahmi Ghrita for managing ADHD, offering a safe, alternative treatment for children suffering from this condition.
