Type  Name  Details 
Intervention  Low-Intensity Pulsed Ultrasound  Alignment and leveling of all teeth will be completed till 0.019" x 0.025" SS archwire. All first bicuspids will be extracted. Two weeks following extraction, closed coil spring will be attached from hook between the lateral incisor and canine to the hook on molar in each quadrant. LIPUS will be delivered immediately after activation of closed coil springs for en-masse retraction following the regimen on 0,3,5,7,14th day in the first month, and thereafter 15th day of every month till space closure (approximately 9 months) for 20 minutes per jaw. The device to be used for the current study, Boneheal 1000 (Technomed Electronics, Chennai), is customized with the US FDA-approved parameters for LIPUS. LIPUS signal consisting of 1.5 MHz frequency, 1 KHz repetition rate, Output intensity 30 mW/cm2, and 200 µs signal burst width with a probe diameter of 25 mm2 will be used. LIPUS application will be performed on was the soft tissue covering of the face over the gingiva and alveolar mucosa in relation to the buccal aspect of teeth (canine to canine) for both jaws (The target site will be the bone surrounding the anterior teeth). 
Comparator Agent  Non-LIPUS  Alignment and leveling of all teeth will be completed till 0.019" x 0.025" SS archwire. All first bicuspids will be extracted. Two weeks following extraction, closed coil spring will be attached from hook between the lateral incisor and canine to the hook on molar in each quadrant. The retraction will then begin without LIPUS application and coil springs will be reactivated every month and space closure will be assessed.

Details  Patients with Class I malocclusion with bimaxillary protrusion.
Average growth pattern of the jaws.
Patients within the age range of 16-25 years.
Patients with rationally advised extraction of first premolars for orthodontic treatment.
Patients with fully erupted permanent dentition.
Patients with good oral and general health.
 
 
ExclusionCriteria 
Details  Patients with pregnancy, systemic disease, or syndromes.
Medication interfering with tooth movement.
Skeletal discrepancy in any of the three planes.
Extracted, missing, or badly decayed permanent teeth (not including 3rd molars).
History of orthodontic treatment.

Brief Summary   The study is a clinical trial to evaluate and compare the rate of orthodontic tooth movement in patients undergoing en-masse retraction with and without Low-Intensity Pulsed Ultrasound (LIPUS) application.
The patients requiring all first bicuspid extraction are to be included. Retraction is to be performed using NiTi closed coil springs attached from hooks between canine and lateral incisors to hooks on molars.
The amount of space closure is to be recorded every month by measuring the space remaining between canine and second bicuspid in all four quadrants.
