Intervention  Group A (Control Group n=15): Will be treated with Connective Tissue Graft using Coronally Advanced Flap Technique.   Langer and Langer2first described the use of connective tissue grafts (CTG) covered by coronally advanced flap for the treatment of gingival recession. The subepithelial connective tissue graft, because of its high predictability in achieving root coverage, has been considered the gold standard of soft tissue grafting procedures. This technique, however, has the disadvantage of requiring an additional surgical site to harvest the CTG, with its associated morbidities and complications.  
Comparator Agent  Group B (Test Group n=15): 15 sites of Miller’s Class I and Class II recession will be treated with Periosteal Pedicle Graft Using Coronally Advanced Flap Technique.  AjayMahajan3 developed the periosteal pedicle graft (PPG) technique.3This technique involves the use of periosteal tissue as a graft material to cover denuded root surfaces along with a coronally advanced flap. The rationale behind the use of periosteum as a pedicle graft is its high vascularity, osteogenic potential, and an abundance of fibroblasts, osteoblasts and stem cells, all of which promotes regeneration.

Details  1. Patients with localized multiple gingival recession (Miller Class I or II) on two contralateral sites of the maxillary or mandibular arch with a minimum 2-3 mm recession depth.
2. Age range: 18-50 years.
3. Systemically healthy patients with no contraindications for periodontal surgery.
4. Plaque Index Score (Silness J and Loe. H, 1964)less than 1 post-phase I therapy.
 
 
ExclusionCriteria 
Details  1. Pregnancy or lactation.
2. Systemic diseases affecting treatment outcome.
3. Patient using tobacco in smoke or smokeless form.
4. Alcoholics and patients with history of drug abuse.
5. Use of antibiotics or long-term anti-inflammatory medication within the past three months.

Brief Summary   This randomized clinical study aims to compare the effectiveness of Periosteal Pedicle Graft (PPG) and Connective Tissue Graft (CTG) in combination with the Coronally Advanced Flap (CAF) technique for treating Miller’s Class I and II gingival recession. Thirty systemically healthy patients, aged 18-50, will be recruited and randomly assigned to two groups: Group A (CTG + CAF) and Group B (PPG + CAF), with 15 participants in each. Preoperative care includes scaling and root planing, followed by reevaluation after 4-6 weeks. Clinical parameters such as plaque index, gingival index, probing depth, and recession depth will be recorded at baseline, 3 months, and 6 months postoperatively. The study will assess root coverage, keratinized tissue width, recession coverage, and esthetic outcomes. Data will be analyzed using SPSS software, and the study will follow ethical guidelines with informed consent from all participants. The objective is to determine if there are significant differences in clinical outcomes between the two grafting techniques in terms of root coverage and periodontal health.
