Pradhan karma (operative) 1 lubricated catheter will be inserted upto the 4.5cm into the anal region parallel to spine. 2 By gently pushing the oil by applying uniform pressure , patient will be asked to take slow deep breath 3 catheter will be removed slowly. Paschat karma (Post operative) 1 patting/massaging of the buttocks will be done. 2 Patient will be asked to sleep in supine position for 10min )

Patient with uncontrolled hypertension, diabetes mellitus
Pregnant women, lactating mother and during menstrual cycle
Patient having any ano-rectal bleeding, fistula, adhog raktpitta
Patient with known case of HIV, HBSAG, CKD, liver failure

Brief Summary    According to modern science colchine given the management of acute gout. The most common adverse effects of this drugs are nausea, vomiting and diarrhoea. 
Patient with recurrent episode can keep supply of analgesics, NSAID’S, cox inhibitor and xanthine oxidase inhibitors etc. local NSAIDS are effective but used less commonly since many patients affected by acute gout have co existing cardiovascular, cerebrovascular or CKD. In  chronic patient of gout, the ayurvedic management will be helpful. Since ayurveda is holistic science, it has better approach of treatment for gout according to patient
Acharya yogratnakar has mentioned administration of ruksha or mrudu basti karma in the management of vatarakta. Basti karma is consider as one of the best treatments for vatrakta. 
so, basti of balaguduchyadi taila and guduchi taila is used and its compartive effect will be monitored on patient of vatarakta
