Cleaning and draping of the perianal region with all aseptic precautions will be done, and we will cover the local area with a towel. 3. Local anaesthesia will be given. 4. Later gloved finger of right hand of the surgeon will be gently introduced into the rectum. 5. A slit proctoscope will be inserted gently to visualise the internal opening clearly. 6. Then a suitable selected probe will be passed through the external opening of fistula. Then the tip of the probe was forwarded along the path of least resistance and will be guided by the finger in rectum to reach into the lumen of anal canal through the internal opening and its tip will be finally directed to come out of anal orifice. 7. Then a suitable length of Nimba KsharaSutra According to the will be taken and threaded into the eye of probe. and then the probe will be withdrawn, leaving behind thread throughout the entire fistulous track. 8. The Two ends of thread will be tied outside the anal canal.)

Brief Summary  
The Fistula usually originates from a perianal abscess in the Intersphincteric space of the anal canal from infection of the anal gland as the anal gland is situated deep to the internal sphincter to open in the Crypts of Morgagni situated on the dentate line. stasis and secondary infection lead to abscess formation and open into the anal canal and the abscess usually tracks down and opens in the perianal skin externally thus Fistula-in-Ano is formed. 

Fistula-in-Ano is a track that has 2 opening external and internal. External opening is considered a secondary opening present at the perianal of the skin while Internal is considered a primary opening present in the anal canal. 
The study will be conducted on 92 diagnosed patients of Bhagandar I.e., Fistula-in-Ano.
