Laryngeal Mask Airway Blockbuster |
Insertion of Laryngeal Mask Airway(LMA) Blockbuster and noting the Oropharyngeal Leak Pressure (OLP) in different head and neck postions for comparison .OLP will be determined by closing the expiratory valve of the circle system at a fresh gas flow of 3 l/min. The airway pressure will be permitted to increase until it reaches equilibrium (a maximum pressure of 40 cm H2O. The equilibrating airway pressure will be recorded when an audible leak occurs over the mouth. The leak pressure shall be recorded and documented at the following position - neutral, head extension-(45° from neutral), flexion (45° from neutral) in random order, followed by rotation to either side. The angle at each position is measured measured using a goniometer.
The patient head will then be placed in the neutral position and surgery will proceed, the LMA will be removed at the end of surgery when the patient is awake. |

Brief Summary
|
The
second generation supraglottic
airways have become the standard
of care for positive pressure ventilation .
The leak pressures determine their efficacy for ventilation and
safety. The evaluation of leak pressures
in different head and neck position will
increase their clinical utility and avoid Endotracheal Tube placement for all surgeries involving changes in
head and neck position. The patient
shall benefit from smooth emergence, decreased requirement of narcotics and
anesthetic agents and avoids airway
instrumentation. |
