Comparator Agent  remote ischemia preconditioning  Before each HD session, a standard blood pressure cuff will be placed around the arm that did not have vascular access, usually the right arm. In the control group, the cuff is inflated to 50-mmHg pressure for 5 min, followed by 5 min of deflation to allow reperfusion. This procedure is repeated three times during a hemodialysis session. it will be done for 12 consecutive emodialysis session thus lasting study for a period of one month 
Intervention  Remote ischemia preconditioning  Before each HD session, a standard blood pressure cuff will be placed around the arm that did not have vascular access, usually the right arm. In the intervention group, the cuff is inflated to 200-mm Hg pressure for 5 min, followed by 5 min of deflation to allow reperfusion. This procedure is repeated three times during the initial period of hemodialysis lasting 30 minutes. it will be done for 12 consecutive hemodialysis session,thus having a stidy duration of 1 month

ExclusionCriteria 
Details  Patients who had acute coronary syndrome, stable angina pectoris, or decompensated heart failure in the 6 months before inclusion
Overt cardiac failure: NYHA CLASS 3 and CLASS 4
Patients with cerebrovascular disease, chronic liver disease, gastrointestinal bleed, malignancy
Current smoker
Patients with cardiac event during study period
Patients developing intradialytic hypotension requiring inotropic support

Patients not willing to participate in study

Brief Summary   Chronic kidney disease patients on maintance hemodialysis are at increased risk of cardiovascular disease. Mortality occurs in these patients due to cardiovascular diseases. Even during hemodialysis, these patients are prone to myocardial injury which can affect their cardiac health. all measures which can reduce the myocardial injury in these group of patients have to be sought. remote ischemia preconditioning, which is easy to perform, is considered to be one such measure. this study aims to look for the benefit of remote ischemia preconditioning intervention in reducing myocardial injury in hemodialysis patients.
